The RPA Intensive Care Service (RPA ICS) has 49 beds spread over 4 units within the service. It is the busiest service in Australia, averaging 3500-3900 admissions per year for the past 10 years.

It is the liver transplant centre for NSW; the statewide default referral centre for Extended Clot Retrieval (ECR) and interventional neuroradiology; and is the tertiary referral hospital for Western NSW and the Far West districts. It also provides a Virtual ICU to Broken Hill Hospital.

The ICS is part of the NSW state extra-corporeal membrane oxygenation (ECMO) retrieval service and was in the top 3 of ECMO service providers for Australia for 2021-2022. A large organised ECPR program is part of the core work of the RPA ECMO team.

In addition, the RPA ICS provides specialty cardiothoracic surgical care, neurocritical care, and intensive care support to complex medical, surgical and trauma patients.

The NSW Government has committed $750 million to the redevelopment of the RPA Hospital, which will include an expanded and enhanced Intensive Care Unit. The redevelopment is expected to be completed by 2027.
